Use this Character Reference Form to provide information to Zion Fellowship regarding a Credential Applicant.

1) Please download the Character Reference Form:

2) Please print the Character Reference Form or complete it by converting to a form and then a pdf.

3) Complete the Character Reference Form. Please print clearly in pen. You may also wish to compose a reference letter to include with the Reference Form if you desire to provide more information. 

4) Postal mail, Fax, or email the Form (with letter if you wish) directly to Zion Fellowship at the address on the form. Do not give the applicant the Reference Form, as it is to remain confidential.
